These are the medical forms required by the state of Connecticut. All campers must print and bring a physical copy of each form at registration check-in. If you have any questions please contact us.
Medical Forms
Preparing for your adventure at Tidal River Christian Camp is an exciting part of your camp experience. We prioritize the safety and well-being of all campers. To ensure a positive and wholesome camp environment and to keep the focus of the week on GOD, we kindly ask that you refrain from bringing the following items to camp:
We PROMISE this is not to make your life difficult. In order to run TRCC each summer we must comply with all state laws regarding summer camps.
Due to Connecticut State laws every camper is required to have these forms filled out. Campers must have these forms with them at registration check-in. If they do not have these forms they are not legally allowed to attend TRCC and we will not be able to let them stay.
While we try to have a printer available at registration for emergencies, do not depend on that. You need to have these forms printed and completed prior to camp. Make sure you prepare in advance as some will require a physician's signature.

This is the complete packet of all required Medical Forms campers need to have to attend Tidal River Christian Camp.
With the exception of the Plan of Care Form, every form must be filled out and brought with your camper to registration check-in.
The Plan of Care Form is only required for campers who have a special health care need or disability and it is necessary that special care be taken or provided
while at camp.
The Insurance and Authorizations Form is required for every camper attending any week of Tidal RIver Christian Camp. It is included in the Full Packet. This is the individual form.

The Over The Counter Form is required for every camper attending any week of Tidal RIver Christian Camp. It is included in the Full Packet. This is the individual form.
The Prescriptions Form is required for every camper attending any week of Tidal RIver Christian Camp. It is included in the Full Packet. This is the individual form.
An individual plan of care is necessary
when a child has a special health care need or disability and it is necessary that special care be taken or provided while the child is at Tidal RIver Christian Camp. It is included in the Full Packet. This is the individual form.

Health/Saftey
TRCC will require all staff and campers to sign a form documenting they either:
- Have been fully vaccinated with one Johnson and Johnson or two Phizer or Moderna shots;
- Have tested negative for COVID within the last 24 hours;
- Have tested positive for COVID within the past 90 days with 5 or more days having passed since the positive test and they have been symptom free for 24 hours.
